Privacy notice and disclaimer
Plain-language terms for using this website, what the contact form collects, and what this site is not for.
Medical disclaimer
The content on this website is general information about psychiatric care and about this practice. It is not medical advice, it is not a diagnosis, and it is not a substitute for an appointment with a qualified clinician who knows your history.
Reading this website, sending a message through it, or booking a consultation does not create a physician–patient relationship. That relationship begins only when Dr. Ibeh has accepted you as a patient and care has formally started.
This site is not for emergencies
This website and its contact form are not monitored outside office hours and are never monitored for emergencies.
If you are in immediate danger, call 911 or go to your nearest emergency room. If you are in crisis or thinking about harming yourself, call or text 988 (the Suicide & Crisis Lifeline) or text HOME to 741741 (Crisis Text Line). All three are free and available at any hour.
What the contact form collects
The contact form asks for your name, email address, an optional phone number, the reason you are getting in touch, how you heard about the practice, and a short message. That information is emailed to the practice so someone can reply to you.
Please do not include detailed medical or health information in the form. Web forms and ordinary email are not fully secure channels, and a standard email inbox is not an appropriate place for protected health information. Once you are a patient, clinical communication moves to a private, HIPAA-compliant channel.
Form submissions are sent by email and are not stored in this website’s database.
Cookies and analytics
This website sets no advertising or tracking cookies of its own.
The booking calendar on the contact page is embedded from Calendly, a third-party service. When that section loads, Calendly may set its own cookies and receive your IP address, governed by Calendly’s privacy policy rather than this one. If you prefer not to use it, you can book by phone or email instead.
How your information is used
Information you send through this website is used only to respond to you and, if you become a patient, to provide care. It is not sold, rented, or shared for marketing.
Once you are a patient, your protected health information is handled under HIPAA and under the practice’s Notice of Privacy Practices, which is provided separately at intake.
Your choices
You can ask what information the practice holds about you, ask for it to be corrected, or ask for it to be deleted where the law and medical record-retention rules allow. Write to the practice at ibehoffice@gmail.com or call (732) 333-3779.
Licensure and scope
Dr. Khadija Ibeh, MD, MPH is licensed to practice medicine in New Jersey and New York. Care can only be provided to patients who are physically located in one of those two states at the time of the appointment.
The practice serves adults only.
External links
This website links to Psychology Today, Calendly, and crisis services. Those organisations run their own websites under their own privacy policies and terms, and this practice is not responsible for their content.
